'poetry' can be an answer for 'form' (I have seen 'Form of literature ' mean 'poetry' so perhaps 'form' could also mean 'poetry'). I'm not sure about the 'an art' bit.

'author to attempt' is the wordplay.
'author' becomes 'Poe' (Edgar Allan Poe).
'to attempt' becomes 'try' (synonyms).
'poe'+'try'='POETRY'
