I cannot quite see how this works, but
'blame' could be 'rap' (synonyms) and 'rap' is found within the answer.
'accepted' could be 'a' (financial abbreviation) and 'a' is found within the answer.
'doctor' could be 'gp' (general practitioner) and 'gp' is found in the remaining letters.
This accounts for all the letters.
This may be the basis of the clue (or it may be nonsense).
