'lady also' becomes 'she' (I can't justify this - if you can you should give a lot more credence to this answer).
'capsized' says the letters should be written in reverse (in a down clue, the word turns upside down).
'and' becomes 'too'.
'trapped inside' is an insertion indicator.
'too' back-to-front is 'oot'.
'she' enclosing 'oot' is 'SOOTHE'.
