I cannot quite see how this works, but
'man's' could be 'his' (belonging to a man) and 'his' is present in the answer.
'doctor' could be 'mo' (medical officer) and 'mo' is found in the answer.
The remaining letters 'mac' is a valid word which might be clued in a way I don't understand.
This may be the basis of the clue (or it may be nonsense).
