'clear out' means to remove the middle letters (I've seen 'cleared out' mean this).
'getting' becomes 'collar' (collar is a kind of getting).
'rid of small' becomes 'am' (I am not sure about this - if you are sure you should give a lot more credence to this answer).
'collar' with its centre taken out is 'cr'.
'cr'+'am'='CRAM'
