'doctor' indicates anagramming the letters.
'framing' means one lot of letters goes inside another (inserted letters are framed by the outside letters).
'what' becomes 'pen' (I can't justify this - if you can you should believe this answer much more).
'seuss' is an anagram of 'susse'.
'susse' going around 'pen' is 'SUSPENSE'.
