I believe the answer is:
neglect
This clue might be a double definition.
'failure' is the first definition.

'lack of maintenance?' is the second definition.
'neglect' can be an answer for 'lack' (I have seen 'Lack of care' mean 'neglect' so perhaps 'lack' could also mean 'neglect'). I am not sure about the 'of maintenance?' bit.
