I believe the answer is:

err

'go wrong' is the definition.
(I've seen this in another clue)

'or right after some hesitation' is the wordplay.
'right' says to take the final letters (the letter on the right of the word).
'after' says to put letters next to each other.
'some hesitation' becomes 'er' (sound made while hesitating - um, er).
The final letter of 'or' is 'r'.
'r' after 'er' is 'ERR'.

(Other definitions for err that I've seen before include "Make mistakes" , "Leave the straight and narrow" , "do something you shouldn't" , "Deviate" , "Be mistaken" .)
