'up' shows that the letters should be reversed in order (in down clue: letters go upwards).
'drip' becomes 'iv' (intravenous drip).
'internally' means one lot of letters goes inside another.
'stop' written backwards gives 'pots'.
'pots' going around 'iv' is 'PIVOTS'.
