'confused' is the definition.
Both the definition and answer are adjectives. Perhaps there's a link between them I don't understand?

'got up with difficulty thoroughly' is the wordplay.
I cannot really see how this works, but
'got' could be 'led' (leading is a kind of getting) and 'led' is found in the answer.
This explanation may well be incorrect...
