'hat back-to-front girl' is the wordplay.
'hat' becomes 'lid' (synonyms).
'back-to-front' shows that the letters should be reversed in order.
'girl' becomes 'emma' ().
'lid' written backwards gives 'dil'.
'dil'+'emma'='DILEMMA'

'in' acts as a link.
