'nurse' can be an answer for 'worker' (I have seen 'Medical worker ' mean 'nurse' so perhaps 'worker' could also mean 'nurse'). I'm not sure about the 'health' bit.

'runs off to spain' is the wordplay.
'off' indicates anagramming the letters.
'to spain' becomes 'e' (abbreviation for Espana).
'runs' with letters rearranged gives 'nurs'.
'nurs'+'e'='NURSE'
