'a doctor' becomes 'dr' (abbreviation).
'to accept' means one lot of letters goes inside another.
'monetary union' becomes 'emu' (I can't justify this - if you can you should give a lot more credence to this answer).
'dr' going around 'emu' is 'DEMUR'.
