'is to meet doctor' becomes 'mo' (I can't justify this - if you can you should believe this answer much more).
'to get' says to put letters next to each other.
'in short' means to remove the last letter.
'snip' with its last letter removed is 'sni'.
'cain'+'mo'+'sni'='cainmosni'
'cainmosni' in reverse letter order is 'INSOMNIAC'.
'as they indicated' acts as a link.
