'runner blocked by short kid' is the wordplay.
'runner' becomes 'miler' (miler is a kind of runner).
'blocked by' means one lot of letters goes inside another (I've seen this in other clues).
'short' means to remove the last letter.
'kid' with its final letter taken off is 'ki'.
'miler' going around 'ki' is 'MILKIER'.

(Another definition for milkier that I've seen is " Less strong".)
