(I know that fellow is a type of man)

'tripped and exclaimed in pain' is the wordplay.
I cannot really understand how this works, but
'in pain' could be 'ow' (I've seen this in other clues) and 'ow' is found within the answer.
The remaining letters 'fell' is a valid word which might be clued in a way I don't see.
This may be the basis of the clue (or it may be nonsense).
