'problem' is the definition.
(eye problem)

'england's first coach conceals very well' is the wordplay.
'england's first' becomes 'e' (1st letter of 'england').
'coach' becomes 'train' (both can mean to instruct a sportsperson).
'conceals' means one lot of letters goes inside another.
'very well' becomes 'yes' (I've seen this in other clues).
'e'+'train'='etrain'
'etrain' placed around 'yes' is 'EYESTRAIN'.

'with vision' is the link.
I am not very happy about this link. Some or all of it may belong to another bit of the clue.

Can you help me to learn more?

(Other definitions for eyestrain that I've seen before include "Optical irritation or tiredness" , "this could require attention from ophthalmologist" , "What may give blurred sight" , "Optic fatigue" , "tiredness of pupils" .)

I've seen this clue in The Times.
