I cannot really see how this works, but
'sick' could be 'ill' (similar in meaning) and 'ill' is found within the answer.
'boy' could be 'page' (a young male servant) and 'page' is found within the remaining letters.
No letters remain.
This may be the basis of the clue (or it may be nonsense).
