drag

'silver' becomes 'ag' (Ag is the chemical symbol for silver).
'following' means one lot of letters go next to another.
'doctor' becomes 'dr' ('Dr' can be a synonym of 'doctor').
'ag' after 'dr' is 'DRAG'.
