'at' means one lot of letters go next to another.
'flexible' becomes 'plastic' (I've seen this in another clue).
'gp's office' becomes 'surgery' (I can't justify this - if you can you should believe this answer much more).
'plastic' put next to 'surgery' is 'PLASTIC SURGERY'.
