(I've seen this in another clue)

'ruffian' becomes 'yob' (synonyms).
'upset' shows that the letters should be reversed in order.
'small' becomes 's' (abbreviation - e.g. clothes size).
'yob' backwards is 'boy'.
'boy'+'s'='BOYS'
