'ragusa' can be an answer for 'town' (I have seen 'Sicilian town' mean 'ragusa' so perhaps 'town' could also mean 'ragusa'). I am not certain of the 'a sicilian small' bit.

'kid going to america' is the wordplay.
'kid' becomes 'rag' (I've seen this in other clues).
'going to' says to put letters next to each other.
'america' becomes 'usa' ('U.S.A.' can be a synonym of 'america').
'rag'+'usa'='RAGUSA'
