Although both the answer and definition are singular nouns, I cannot understand how one could define the other.

'doctor sounds surprised about' is the wordplay.
'doctor' becomes 'dr' (abbreviation).
'sounds surprised' becomes 'ahs' (I can't justify this - if you can you should give a lot more credence to this answer).
'about' says the letters should be written backwards.
'dr'+'ahs'='drahs'
'drahs' reversed gives 'SHARD'.
