Leaving nursing home, sign on form for discharge (5)
I believe the answer is:
rheum
'for discharge' is the definition.
(I've seen this before)
'leaving nursing home sign on form' is the wordplay.
I cannot really see how this works, but
'home' could be 'h' (abbreviation as used in sports fixtures) and 'h' is found in the answer.
'on' could be 're' ('on' can be mean 'with reference to') and 're' is found in the remaining letters.
The remaining letters 'um' is a valid word which might be clued in a way I don't see.
This may be the basis of the clue (or it may be nonsense).
(Other definitions for rheum that I've seen before include "that's rather snotty?" , "nasal drops" , "nasal discharge" , "Discharge of water" , "runny nose" .)
