Although both the answer and definition are singular nouns, I cannot understand how one could define the other.

'papa poorly?' is the wordplay.
'papa' becomes 'P' (phonetic alphabet: alpha, bravo, charlie etc.).
'poorly?' becomes 'ill' ('ill' can be similar in meaning to 'poorly').
'p'+'ill'='PILL'
