'sickly' is the definition.
(I've seen this before)

'uneasy state getting off ship with bag in front' is the wordplay.
I cannot quite see how this works, but
'state' could be 'ri' (Rhode Island abbreviation) and 'ri' is found in the answer.
'bag' could be 'sac' (I've seen this before) and 'sac' is present in the answer.
This explanation may well be incorrect...

Can you help me to learn more?

(Other definitions for saccharine that I've seen before include "China cares (anag.)" , "Excessively sweet" , "Sweetener" , "Excessively sentimental" , "Sickly" .)

I've seen this clue in The Telegraph.
