('lean' can be the opposite of 'fat?' so perhaps 'leaner' is reasonable for 'less fat?')

'somebody with inclination' is the wordplay.
'somebody' becomes 'er' (I can't justify this - if you can you should believe this answer much more).
'with' means one lot of letters go next to another.
'inclination' becomes 'lean' (synonyms).
'er' put after 'lean' is 'LEANER'.
