'where' is the definition.
Both the definition and answer are adverbs. Perhaps you can see an association between them that I don't see?

'nurses may be offering direction' is the wordplay.
I cannot quite see how this works, but
'direction' could be 'nw' (NW is an example) and 'nw' is present in the answer.
This may be the basis of the clue (or it may be nonsense).
